Complaints & Concerns
This form is to assist you in making a complaint (or expressing a concern) to our organization. All persons wishing to do either of these may speak directly with the Agency Coordinator (250) 372-0179 or by completing the form online or downloading the Client Complaint Form and mailing or dropping it off at the Centre. All information is strictly confidential and will only be viewed by the Agency Coordinator.
If you would like help completing either the electronic form or the paper format, please call the Centre and ask for the Agency Coordinator/Privacy Officer. We encourage you to express your concerns in writing.
